Before
The line between cosmetic and reconstructive plastic surgery can sometimes be quite fine. This 65 year old lady had undergone a gastric sleeve procedure for weight loss and dropped almost 75 pounds. This left her with a lot of loose abdominal skin and a bulging abdomen due to weak muscles. Her situation was such that I felt she needed both a horizontal and a vertical incision to remove as much skin and fat as possible, something I call an extended abdominoplasty. She also needed to have the muscles tightened. I proposed this to her and she accepted without hesitation.
After
She is shown six months after surgery. The result was quite dramatic and she was very happy, as was I. She has nice, faint scars. It is not a perfect result, but neither of us expected that, given her starting point. In these surgeries expectations are critical and must be realistic.
