Patient - Breast Lift

Before

This young woman had a breast augmentation by another surgeon 15 years ago. She did well, but over time experience a change in the left breast, which look too bottom heavy and the implant shifted almost into her armpit when she would lie on her back. Her implants were saline implants and her card indicated they were 375 cc Mentor smooth, round, moderate profile implants. The right was filled to 395 cc and the left to 425 cc. When I examined her, she had a lovely long term result on the right and a hypermobile implant on the left. She had relatively little breast of her own and was mostly implant. I suspected her capsules- the scar tissue around her implants- would be very thin and filmy and the stress of the larger implant in the smaller left breast accounted, at least in part, for the change over time of this breast. I proposed a change to gel implants and revision of her left implant pocket.

After

At the time of surgery, I confirmed that her implant capsule was as thin and clear as cellophane. I closed down the pocket laterally and inferiorly with sutures, then reinforced the repair with two sheets of GalaFlex surgical scaffold. The scaffold material will protect my suture line and, as it slowly breaks down, will be replaced by her own scar tissue. I did a straight implant exchange on the right. Her new implants are Allergan Natrelle Inspira Style SRM smooth, round gel implants. The right implant is 445 cc and the right one is 485 cc. Her photos show improved shape of the left breast and better symmetry with the right, albeit not perfect. This type of breast revision is, in my opinion, one of the most difficult in which to obtain satisfactory results. I was satisfied and so was she.
